Student and Family Information Forms
The following forms NEED to be completed each school year. Please take some time to click on each link to complete the required documentations.
This following form is to be completed by families registering their child in a Connecticut public school for the first time. Regardless of their home or dominant language, all students must have a completed "Home Language Survey" in their school records upon enrollment. If you have never filled out such a form or if your child has never been enrolled in a CT public school, click on the following link:

Families who are unable to complete the “Student and Family Information Form” online are required to complete the forms included below on paper and return them with your child during the first week of school.
