Health Requirements

 

It is good practice to submit a physical every year for your scholar.  However, in PK, Kindergarten and 6th grade, physicals are mandatory.  Please have your child's pediatrician fill out one of the forms below.

 

If your scholar requires medication during school hours, the school health office MUST have an authorization on file.  This allows us to safely and legally give the medication.  Please have your child's pediatrician or specialist fill out the form below for EACH medication required.

PLEASE NOTE: Medication should not be carried by any student for any reason.  This is a major safety risk.  All medication should be transported by an adult.

Also, a new authorization form is needed EACH school year.

 

If your child suffers from intermediate or persistent asthma, it is helpful to have an action plan on file in the health office.  This action plan lets anyone in care of your child know exactly what to do in case their symptoms flare up.  Please have your child's pediatrician fill out the action plan along with an authorization form for their asthma rescue inhaler, also known as Albuterol.
