Question #3: Photo/Media Release, 2025-2026 (Pregunta 3: Foto/Formulario de publicación de medios 2025-2026 Año Escolar)

I hereby give permission for New Beginnings Family Academy to photograph, videotape, record or otherwise capture the voice and likeness of my child in connection with marketing, publicity or public relations projects to benefit the school.

I grant New Beginnings Family Academy all rights (including copyright ) in all recordings, photos, videotapes and digital media of my child, including the right to reproduce, exhibit, broadcast, or distribute such materials without limitation(including, but not limited to, purposes of education, advocacy and fundraising).

I give permission for New Beginnings Family Academy to use my child’s name and likeness in press releases to news media and that the media outlets may or may not elect to include my child in any coverage of New Beginnings Family Academy.

I release New Beginnings Family Academy from any claims of any kind relating to use of any of these recordings, photos, videotapes, films and digital media including, but not limited to, claims for copyright infringement or invasion of right of privacy or publicity.

I agree to inform New Beginnings Family Academy immediately if my child is no longer allowed to be photographed, videotaped, or recorded to promote New Beginnings Family Academy.

By signing below, I represent that I have read and approved this release and I have legal authority to sign this document on behalf of my child.

Yo doy permiso para que New Beginnings Family Academy fotografíe, grabe, o capture la voz y semejanza de mi hijo en relación con proyectos de marketing, publicidad o relaciones públicas para beneficiar a la escuela.

Question #4: Field Trip Permission Form, 2025-2026 (Pregunta 4: Formulario de permiso de viaje 2025-2026 Año Escolar)

I hereby give my child permission to accompany New Beginnings Family Academy on assorted field trips and activities outside of the school building throughout the 2025-2026 academic year.  I understand that by signing this form I am allowing my son/daughter to be transported by school bus or on foot to any day trips including, but not limited to theaters, parks, stadiums, the state capitol, zoos, museums, outings, etc.  I understand that NBFA personnel will arrange transportation to and from the field trip, if transportation is needed.

I understand that some trips may require a small contribution to help defray the costs.  In those instances, my child will receive a separate notice.

In the event that medical personnel must be called to attend to my child during a field trip, I understand that the school will make every effort to contact me or an authorized representative immediately.  However, if I am unable to be reached, I give the school permission to authorize medical help for my child in my absence.

I agree to inform the school immediately if my child is no longer allowed to attend any day trips arranged by New Beginnings Family Academy.

Por la presente, doy permiso a mi hijo para acompañar a New Beginnings Family Academy en una variedad de excursiones y actividades fuera del edificio escolar durante el año académico 2025-2026. Entiendo que al firmar este formulario, permito que mi hijo/hija sea transportado en un autobús escolar o a pie a cualquier excursión de un día, incluidos, entre otros, teatros, parques, estadios, el capitolio estatal, zoológicos, museos, excursiones, etc. Entiendo que el personal de NBFA organizará el transporte hacia y desde la excursión, si se necesita transporte.

Entiendo que algunos viajes pueden requerir una pequeña contribución para ayudar a sufragar los costos. En esos casos, mi hijo recibirá un aviso por separado.

 En caso de que se deba llamar al personal médico para atender a mi hijo durante una excursión, entiendo que la escuela hará todo lo posible para comunicarse conmigo o con un representante autorizado de inmediato. Sin embargo, si no puedo ser contactado, doy permiso a la escuela para autorizar ayuda médica para mi hijo en mi ausencia.

Acepto informar a la escuela de inmediato si mi hijo ya no puede asistir a ningún viaje de un día organizado por New Beginnings Family Academy.
